Placard & QR Placement and Nominations

To request a placard or QR code to attach to a headstone, please email to the link below and provide information such as photo capture of obituary, family photos, family stories, and any notable accomplishments. I request a donation (donate button on the bottom of this page) depending on the type of Dash Capture requested.
Choose which ones you are requesting to have created - to request all three, it would be a requested $45 donation:

QR Code for Headstone ($5 requested donation)
Small Placard for QR Code ($15 requested donation)
Large Placard with Pictures, Information and QR Code ($25 donation)
